Shared information key to shared care

Everyone agrees that frontline healthcare professionals need systems to share clinical and demographic information between teams so that shared care can be practised safely and efficiently. Achieving that desired flexibility and smooth communication has proved to be difficult in reality.

The good news is that it can be done. This feature, sponsored by Graphnet, brings together some practical examples of information sharing in NHS trusts in Salford and South Staffordshire. Both trusts have worked closely with Graphnet to ‘bridge the gap’ and bring together information from diverse sources that can be shared by healthcare professionals.

The projects used Graphnet’s solutions:
